Advanced Care Planning
Advanced care planning refers to the process of discussing and documenting an individual's preferences for their healthcare, including their wishes regarding end-of-life care, as they age or face serious illness.
At the Heartfelt Legacy Foundation, Our mission is to normalize advanced care planning conversations within our local community, bringing together families, healthcare providers, and other professionals to openly share and discuss the wishes of individuals as they age, navigate life's transitions, and consider what end-of-life care means to them. We aim to create a culture where these conversations are seen as essential, helping families and professionals make informed decisions that reflect the values and desires of each individual.
Special Needs Families
The Heartfelt Legacy Foundation is dedicated to supporting families of people with disabilities and children with special needs by connecting them with the professionals they need and providing up-to-date education through seminars and workshops on key topics, such as legal changes and industry developments.
We collaborate with nonprofit organizations, government agencies, and the legal and financial planning communities to bring in nationally recognized experts in special needs planning each year. These events empower both families and local professionals to make informed decisions that benefit the well-being and future of individuals with special needs or disabilities.
Our goal is twofold: to connect families and caregivers with the right resources and professionals, and to provide education that keeps families and professionals informed and equipped. By fostering these connections, we ensure families receive the support they need while strengthening the network of professionals dedicated to our community.
